Spring Sports Participation Information
All Athletes must be cleared to participate by the nurse’s office before they can practice. JV and Varsity students, and modified basketball candidates, who have not been cleared to participate in a winter sport must visit the nurse’s office between 7:45 am and 3:15pm.
Students must report directly to the Notre Dame school nurse’s office. Any athlete may drop off/mail their documentation to the main office, if they have it ready before these sessions. If you are doing this, please ask the staff to put the documents in the nurse’s' mail box. Do not give paperwork (health history forms, and documentation of physical exam) to coaches. Only the school nursing staff can clear students to participate.
Paperwork that must be submitted by athletes:
· Pre-participation health history form and concussion form. Both forms must be complete and signed by parent/guardian and the student.
· Documentation of a physical examination. To be done by a physician, physician assistant or nurse practitioner on or before:
August 1, 2017 (Fall Sports)
October 1, 2017 (Modified Boys Basketball)
November 1, 2017 (Winter Sports)
March 1, 2017 (Spring Sports).
Note: No physicals will be done at school. Athletes are responsible for getting their own physicals and bringing in documentation. The only exception is for students who are new entrants to NDMS/HS, from a district other than Elmira, for the Fall.

Students in 7th or 8th grade who are especially talented may be eligible to compete on a Junior Varsity or Varsity team. These students must pass the selective classification criteria established by the NYSPHAA.
