At Notre Dame we understand that the health of our students directly impacts their ability to be successful in school. We work hard to ensure that all students are healthy and that we are vigilant about attending to any health issues. Our school nurse is readily available to students during school days to tend to injuries or illness that occur.
From the Nurse:
For any student needing medication at school, they must bring a Doctor's note to the school nurse. If they do not bring a note, they will not be able to take their medication on school grounds.
All new entrants need a current physical before being able to attend school. Physicals can be done by Primary Doctors and sent to the school nurse when completed.
Students are expected to be up-to-date on their immunizations. Documents of these immunizations should be sent to the school within two weeks of having them done, or they will not be valid.
Emergency and Health History Forms Must be Up-To-Date. It is important that parents/guardians complete the Emergency and Health History Forms that are sent home with their student at the beginning of each school year. These forms give us accurate contact, consent and health information about your child that is needed by the health office. Also, please keep the Main Office up-to-date with phone numbers and emergency contacts.
